Complete the online enrolment form below so Araya can understand the student’s details, class level, contact details, permissions and support needs.

Who can enrol

This form can only be completed and submitted by the student’s parent or carer, and you must be 18 years of age or older to submit it. If you are not the student’s parent or carer, please contact Araya first so we can guide you through the right process.

Privacy notice — before you begin

This form collects personal and sensitive information about the student and family, including contact, medical, allergen, dietary and emergency contact details, so Araya can safely plan, supervise and run the program. This information is used only for enrolment, safety and program administration, is stored securely, and is shared only with Araya staff and volunteers directly involved in your child’s care, the service providers that run Araya’s systems (such as website hosting), emergency services when needed, and authorities where the law requires it (for example, reporting child safety concerns). Araya does not sell your information, uses photos only if you give permission below, and keeps it only as long as needed for these purposes. Read the full Privacy Policy for details on what is collected, how it is stored and protected, and how to access or correct your information.

4Class selection

8Permissions and consent

I confirm I am the student’s parent/carer, I am 18 years of age or older, and I have authority to submit this enrolment. * I give permission for the student to participate in selected Araya programs and supervised activities. * I authorise Araya to seek emergency medical assistance if needed. * I give permission for the student to join supervised incursions held at the program venue. I give permission for the student to join supervised excursions away from the program venue. I acknowledge Araya’s child safety expectations, supervision procedures and reporting responsibilities. * I agree to Araya’s Privacy Policy, Terms and Conditions, Refund, Cancellation and Payment Policy, Complaints and Feedback Policy and Website Content and Third-Party Disclaimer. * I give permission for appropriate photos/videos to be used for Araya program updates, promotion or social media.
